What exactly do we mean by “family approach” at The Palms at Ponte Vedra? Familiarity is important when it comes to the comfort of the residents in our Assisted Living/Memory Care center; we know every resident by name, and with our success at staff retention, recognizable faces are in-house around the clock. Regularly scheduled social activities encourage neighborly friendships. Ultimately, folks who reside with us care about and look out for each other, because they know each other. Compassion is contagious at The Palms, and we pride ourselves in the culture we have created here.
Sole reliance on charts and notes in a computer database isn’t good enough for us. Nothing can replace the personal touch of our hands-on techniques. We know behaviors, habits, and physical deficits just by getting to know each person who lives here. Safety, health, and communication are taken seriously. If something is amiss (i.e., unusual behavior, appears to feel under-the-weather, mobility issues, etc.), we notice, thus, we firmly believe in a “see something, say something” policy. Waiting for a family member to approach us about a change in their loved one is unacceptable to us. We engage with our residents and proactively communicate with family members and among our staff, effectively keeping everyone up to date with diagnoses, treatment plans, progress, and changes.
Furthermore, we orchestrate holiday celebrations and recommend family members visit and be involved. Family presence is important when it comes to long-term assisted living and memory care. Family members are welcome any time and encouraged to participate in the many social activities here at The Palms. We even have a private dining room to accommodate a sizeable family gathering for birthdays, life milestones, holidays, or an impromptu visit. Family first!
